                          PH&FC BILLING/SERVICE POLICIES

                                                 Services¹:

             Investment:

1.   In-Person Meetings

Sessions for creating a total health and fitness program that addresses the client’s unique fitness goals, physical limitations, and time/equipment constraints. Includes examination of orthopedic and other health concerns, body composition (bodyfat and/or circumference measurements), and strength training, cardiovascular, flexibility, and dietary needs, if indicated. Written program guidelines are provided. This is a true learning experience designed to give the client a thorough foundation for reaching his/her desired goals!

 

(1) $70/hr (pre-paid in blocks of five sessions); OR

(2) $80/hr (paid at the time of service, $80 refundable retainer fee required)

 

 

2.   Telephone Consultations

Scheduled phone calls with the fitness professional on a designated day and time for a predetermined length of time. Topic(s) determined by mutual consent.

 

$50/hour (minimum charge = ½ hr; minimum pre-payment = $100)

 

 

3.   Assistance in Selection/Purchase of Home Fitness Equipment

(Alternatively or in addition, a nominal, mutually-agreed upon fee may be charged to cover time spent searching for and/or purchasing equipment for a client)

 

 

$60 per hour (minimum charge = 1 hour)

 

 

4.   Other Services/Program Enhancements (ü = provided optionally at no additional charge as part of an in-person meeting package; pre-payment  required):

ü      Body Composition (Bodyfat and/or Circumference) Assessment and Report ($50)

ü      “CardioCheck” Target Heart Range Assessment and Report ($50)

ü      Written “Steady-State” Aerobic Exercise/Fat Burn Plan ($40)

ü      Written “High Intensity Interval Training” Aerobic Exercise/Fat Burn Plan ($40)

ü      Computerized Diet Assessment Report ($50)

·         BodyGem resting metabolic rate test and report ($50)

·         Weight training routine “Picture/Tip Sheet” ($40)

·         Range of Motion (Stretching) “Picture/Tip Sheet” ($40)

·         Written “Pyramid Diet” Point Plan (similar to Weight Watchers; $50)

 

 

*Cancellation Policy*

·         With at least 24-hours notice: No charge for the cancelled session or service; reschedule at a mutually-convenient time

·         With less than 24-hours notice: Cancelled session or service billed at one-half the normal rate; reschedule, if desired, at a mutually-convenient time using the normal session or service rate

·         If a pre-paid training package is prematurely terminated, PH&FC will refund one-half of the value of any unused sessions.

Although PH&FC is thoroughly committed to each of its scheduled client meetings, in the rare event that we need to cancel a session, we will provide at least 24-hours notice. Should we need to cancel with less than 24-hours notice, the meeting will be rescheduled at a mutually convenient time and the session will be provided at one-half of the normal session rate.

Each in-person training session is normally 60 minutes long, although longer meetings may be arranged by mutual agreement. Sessions running longer than the normal 60 minutes will be billed in additional 15 minute increments.

To get the most out of the sessions, clients should be ready to proceed at the appointed starting time as sessions will not be extended to accommodate tardiness without incurring fee adjustments as described in the above paragraph. For the same reason, the client is encouraged to eliminate any avoidable interruptions (telephone calls, etc.) once a session has begun. At the conclusion of each meeting, the client may be asked to sign a form acknowledging participation in the session and authorizing any other requested billable services.
